Custom AVATAR Request

Form Description

This is the form you can use to request a custom avatar drawn by Strkt9NinjA our Clan Artist.

This avatar is drawn by hand on paper. It will be yours and only yours, custom to you. But, it does take time... so be PATIENT.

You must donate a minimum of $15 to get a custom avatar made.

Your REAL NAME:* So NinjA can refer to you by name not PSN, Gamertag, etc.
Clan Members ID:* What is your PS3 Name, XBOX 360 Gamertag, Wii Friends Code, etc.
